How do you verify an AI medical record summary against source records?
Start with the claim, then find the record that supports it. A useful review checks dates, diagnoses, treatments, test results, and source citations—not just whether the summary sounds plausible.

AI can put thousands of pages into a readable narrative quickly. That is valuable. But an attorney needs to know which statements are grounded in the chart and which ones need correction or qualification. Verification is a focused comparison between an existing AI-generated report and the underlying medical records.
Follow the finding back to its source.
For each material finding within the agreed scope, locate the referenced note, result, order, or flowsheet entry. Confirm the date and time, the person documenting it, and what the record actually states. A citation that points to a page is a starting point; it is not proof that the AI characterized the page correctly.
The distinction between an order and an administered medication is a common example. A report may accurately identify a medication order while incorrectly saying the patient received the drug. A reviewer checks the administration record before treating that statement as fact.
Read around the quoted line.
Clinical context can change the meaning of a technically accurate quote. Was a diagnosis preliminary and later ruled out? Did a copied-forward assessment persist after the patient's status changed? Was an abnormal test result addressed in a later note? A source check should account for that sequence, especially where the report draws a clinical conclusion from it.
The result should make the status of each important finding clear: supported, requiring qualification, or unsupported, with a way back to the relevant record. The attorney can then use the AI output with a better understanding of its limits.
Decide whether verification answers the whole question.
Verification evaluates what the existing report says. It does not independently search the record for every significant item the AI may have omitted. If the question is also “What did the report miss?”, the assignment needs an omission review.
